Digital aftercare for at home and on the go

After outpatient or inpatient rehab, it is essential that patients continue their therapy independently. To this end, they receive individually tailored therapy plans that include targeted exercises, behavioral adjustments and medical recommendations. However, the implementation of rehabilitation measures at home is often challenging for those affected, as therapy plans are inflexible, adjustments are time-consuming and the therapist team is only available to a limited extent.

MEDIAN set itself the goal of making this process more efficient and creating a digital solution that maps out customized therapy plans and enables individual care. At the same time, the aim was to manage health data securely and simplify billing with the German Pension Insurance. For this project, MEDIAN found a partner in TWT that understood both the technical and industry-specific requirements precisely.

From analog to digital: rethinking aftercare

With a solution-oriented and partnership-based approach, TWT developed an innovative digital solution for MEDIAN for modern rehabilitation that optimally connects rehabilitants and treating specialists. TWT designed an app including an associated therapy management tool, integrated all relevant features and ensured intuitive use with an appealing design. The MyMEDIAN@Home app offers a digital aftercare program for patients with all somatic indications, is customizable and can be used from any location. 

Users benefit from personalized therapy plans in which they can, for example, perform training therapy exercises, be guided through various relaxation exercises and acquire additional health knowledge through podcasts and videos. In addition, therapeutic experts are always available for personal advice via chat, e-mail or telephone. MyMEDIAN@Home is based on the multimodal concept of IRENA (Intensified Rehabilitation Aftercare) of the German Pension Insurance.

The therapy specialists use the associated therapy management tool to efficiently manage health data and adapt therapy plans. Interfaces and central data management reduce the administrative workload and ensure optimal patient care - in terms of content and time.

The close collaboration between TWT and MEDIAN enabled an intuitive, high-performance solution that combines medical quality with digital innovations. TWT not only realized the technical implementation, but also remains a long-term strategic partner of MEDIAN in order to sustainably improve digital rehabilitation aftercare.

Therapy without interruption

This development makes MEDIAN the first clinic group in Germany with its own digitally supported aftercare service for patients. With in-depth expertise and many years of experience in the healthcare sector, TWT has developed a holistic concept that is specifically tailored to the needs of everyone involved. Thanks to the tailor-made concept and targeted implementation, the app was immediately ready for use and is already being actively used by many patients. Digital aftercare makes it easier to integrate therapy into everyday life, provides long-term motivation and thus promotes sustainable recovery success. At the same time, healthcare professionals benefit from simple networking with their patients, optimized data management and significantly less bureaucracy.
